What is your purpose for taking this training?

What peer certification do you currently hold, if any?

Please upload either your Core Certificate of Completion OR your HHSC Attestation of Completion (www.hhs.texas.gov/providers/behavioral-health-services-providers/peer-support-services/certification-peer-support-a-medicaid-benefit)

I understand that after completion of training(s), I am responsible to apply for certification with the Texas Certification Board to become a Recovery Support Peer Specialist.

I understand it is my responsibility to secure a site for the 250 hours of supervised work or volunteer hours.

By submitting this application, I acknowledge that my role as a Recovery Support Peer Specialist is and must be completely separate from any other role, including that of clinician or sponsor. I also acknowledge that recovery is self-guided and that my role as RSPS may include assisting others on their chosen path, whether that includes abstinence based, medication assisted, harm reduction, or any other path to recovery.
