Case Manager, Cradles (Bilingual)

SUMMARY

This position intervenes with pregnant and post-partum women and women of child-bearing age identified as using substances and having related risk factors (with an emphasis on Opioids) to reduce the incidence of drug exposure of their unborn, newborn and young children, facilitates a healthy full term birth and healthy start to family life for all participants.  Position conducts screenings/assessments, develops strengths based individualized service plans based upon the client assessment, uses Motivational Interviewing, coordinates referrals and services across systems, conducts parenting education and community outreach activities, works in client’s homes and various social service agencies, community-based or medical/health organizations, including Opioid Treatment Substitution sites.

RESPONSIBILITIES

  • Provides screenings/assessments, case management, referral coordination, parenting education, substance abuse education and intervention or identification of critical needs
  • Conducts parent education using appropriate evidenced based curriculum (Nurturing Parenting, Mommies, Becoming A Mom, Seeking Safety)
  • May conduct street outreach in collaboration with other organizations, providing education and materials related to safe sex and hygiene. May conduct pregnancy testing as appropriate
  • Develops and maintains relationships with community partners in various community-based agencies, maternal and child health providers, Medicaid providers, FQHC’s, DFPS and other appropriate providers where screenings and other services will be conducted or who may be referral sources
  • Coordinates services across systems such as maternal/child health providers, DFPS, Medicaid providers, FQHCs
  • Enters all screenings, assessments, case notes, and other funder or agency required data in funder and
  • Council data systems in a timely manner
  • Performs other duties as assigned

JOB REQUIREMENTS

Education

  • Bachelor’s degree in Social Work or other related field
  • MSW or LMSW
  • LCDC strongly preferred

Required Experience:

  • Previous experience working with high risk women and in the substance abuse field
  • Working knowledge of client/community resources and experience in developing relationships with same
  • Experience leading groups and teaching high risk and reluctant client populations

 

Required Skills:

  • Passion for working with high risk women and teens in substance abuse field
  • Strong client engagement skills
  • Strong organizational skills
  • Timely and accurate documentation skills in electronic health records
  • Above average computer skills: Microsoft Word and Excel; CMBHS proficiency preferred
  • Bilingual English/Spanish- REQUIRED
